The superficial part of the dermis (the layer of the skin right beneath the epidermis) is affected in 2a degree scald burns. Family physicians should be alert for psychologic problems related to long-term disability or disfigurement from burn injuries. Second degree scald burns have two sub-stages (2a and 2b) which are very different in terms of outcome. Early surgical referral can often help prevent or lessen scarring and contractures. Complications of burns include slow healing, scar formation and contracture. Follow-up care is important to assess patients for infection, healing and ability to provide proper wound care. Thermal burns from dry sources (fire or flame) and wet sources (scalds) account for approximately 80 of all reported burns and can be classified based on the depth of burn 9, 10. The American Burn Association has established criteria for determining which patients can be managed as outpatients and which require hospital admission or referral to a burn center. Antimicrobials are not required for first degree burns. Some patients may even require hospitalization briefly in order to control pain. Analgesics may be required for patients who experience large first degree burns. In addition, moisturizers may increase comfort. A second-degree burn also causes redness and heat and forms a water blister. A systematic approach to burn care focuses on the six “Cs”: clothing, cooling, cleaning, chemoprophylaxis, covering and comforting (i.e., pain relief).
